Modifier:
A modifier helps to report that a service that has been performed has been altered by some specific circumstance but there has not been any changes in its definition or code.
Modifier 59:
Modifier 59 is used to report if two distinct procedural Service(non-E/M) are performed during the same operative session and one of the two is considered bundled with the primary procedure as per reference given by National Correct Coding Initiative edit (NCCI edit).
Modifier 59 is used to report for the service which are not normally reported together, but are appropriate under the circumstances. If there is already an established modifier which is appropriate it should be used rather than modifier 59. As per CMS it is known as the modifier of last resort. It is the most mistaken modifier that providers often use incorrectly. Medical documentation must support the use of the modifier

● XE – “It is used to describe the services which was performed during the separate encounters on the same DOS.
● XS – “It is used to describe that a service is distinct because it was performed on a separate organ.”
● XP – “It is used to describe that a service is distinct because it was performed by a different practitioner.”
● XU – “It is used to describe that a service is distinct because it does not overlap usual components of the main service.
